[Womoz] Our research on women in FLOSS

Below is the new version of the survey, changed according to the 
suggestions that we have received.

If you have any additional comments, please make sure that you propose 
each time a concrete correction/change so we can edit the survey with it 
directly. Thanks!

We will take into account suggestions until *Monday 18:00 UTC* (so this 
leaves us with a little bit more time than initially announced). After 
18:00 UTC, we'll upload the poll with Limesurvey and start distributing 
it amongst the Mozilla women contributors.
